A menu QR code points a diner's phone straight at your current menu, which is why table cards replaced laminated inserts in most venues. Host the menu as a normal web page where you can, rather than a PDF — a PDF forces pinch-zooming on a phone and loses you orders on the long tail of your list.

How to make a QR code for restaurant menus

Troubleshooting scan failures
Guests complain the menu is unreadable
A PDF is the usual cause. Serve an HTML page that reflows on a phone.
Laminated cards do not scan
Matte lamination instead of gloss, and keep the card upright to reduce glare.
Menu changed but codes are printed
Use a dynamic code and repoint it; the printed pattern never has to change.
Poor signal in the dining room
